The additional Junior form can be found by new starters here on our website or our returning members can find it on our club share drive. But be sure to download the form first and then save it to your local drive with your surname in the filename.

REMINDER - as well as the Club Membership form it is mandatory that ALL Junior Members MUST also complete a consent and medical information form which must be completed and signed in addition to their Club Membership.

We also provide you the option to consent or decline for photos to be used on the club website and all social media channels or in the local newspaper to promote Droitwich Spa Hockey Club through match reports and events.
